What to expect
Please assist us by providing the following information at the time of your consultation:
- Please bring your ID. If the patient is a minor, please bring a parent or guardian ID in addition to the patient's ID If the patient is a minor and accompanied by a guardian, please bring official guardian court paperwork.
- Please bring a list of medications you are presently taking. If you have medical or dental insurance, bring your cards with you. This will save time and allow us to help you process any claims.
- Please bring your referral slip and any radiographs if applicable.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at your consultation visit.
Eating & Drinking
You can eat a regular diet prior to your exam, however please have nothing to eat or drink six hours prior to any procedure done under IV sedation or general anesthesia. You will also need to arrange an adult to drive you home.
Please alert our office if you have a medical condition that may be of concern prior to any procedure or if you are currently taking any medication. Please alert our office if you have any chance of being pregnant.
Radiographs
If your dentist or physician has taken radiographs, you may request that they forward them to our office. Depending on the type of radiograph, the quality of the radiograph and the date of your radiograph, a new radiograph may be necessary and will be taken at our facility.
